LVN Duties and Responsibilities:
  • Receives and provides complete report on all assigned clients.
  • Respects client’s rights and maintains confidentiality.
  • Notifies immediate supervisor of changes in client’s conditions.
  • Understands and uses aseptic technique.
  • Prepares equipment and materials for treatments.
  • Records all findings in clients’ clinical records.
  • Performs other duties as assigned by immediate supervisor.
  • Nursing activities that may include:
    • Assessment.
    • Assistance in planning, evaluating and implementing client care under the supervision of a Registered Nurse.
    • Assisting clients with activities of daily living.
    • Teaching client self care techniques.
    • Providing and maintaining safe environment for client.
    • Accepting responsibility of assignment to specialty area only when training has been provided.
    • Performing within the limits of preparation and experience.
    • Observing aseptic techniques when dealing with equipment and supplies used for treatment.
    • Assisting with client admission and discharges.
    • Notifying charge nurse or supervisor of changes in client’s condition or unusual symptoms.
  • Provides physical comfort and sees to emotional, physical and safety needs of client.
  • Assists in rehabilitation of clients.
  • Accepting accountability for actions while on duty.
  • Maintaining competency through continuing education.
  • Performing all nursing procedures for which trained and prepared.
  • Safeguarding equipment and supplies.
CNA Duties and Responsibilities:
  • Responsible for all duties in relation to safely providing for the resident’s care and well-being.
  • Maintains the facility’s / community’s philosophy.
  • Is knowledgeable of policies and procedures.
  • Be aware of concerns and report any changes immediately.
  • Understands and agrees to follow the resident’s care plan as directed by supervisor.
  • Assist with all of the care needs a resident might require including, but not limited to toileting, ambulation, bathing, dressing, personal hygiene, escorting, and transferring from place to place.
  • Have good communication skills.
  • Maintains confidentiality with resident and fellow employee’s information.
  • Appropriate phone etiquette.
  • Notifies management of resident changes of condition.
  • Assist in the dining room during meal times including, but not limited to, set up, take meal/beverage orders, serve, bus tables.
  • Assist with activities when required and/or be responsible for getting the residents to scheduled activities.
  • Responsible for getting the resident ready and to the lobby for scheduled appointments.
  • Will be aware of what constitutes a change of condition and report to management immediately.
  • Delivers and re-stocks the resident’s incontinent supplies.
  • Has knowledge of, and can demonstrate proper technique, or can learn how to assist with equipment including: A gait belt, transfer board, transfer pole, wheel chair, scale, and other equipment as directed.
  • Capable of taking accurate vital signs and weights
  • Accurately complete an incident report.
  • Aware of and can perform assigned housekeeping duties including, but not limited to, dusting, vacuuming, polishing, wiping down hand rails, chairs, and tables, picking up magazines, cleaning bathrooms, re-stocking toilet and paper towels and mop.
  • Maintains general cleanliness of the resident’s apartment (clean commodes, hi-rise toilet seats).
  • Reports any safety issues in resident’s apartment such as frayed cords, broken furniture/equipment, etc.
  • Notifies management when incident or error occurs while on shift.
  • Reads the shift-to-shift communication and appropriately documents or reports of any duties left for the next shift.
  • Capable of assisting in care needs of residents, which might include lifting requirements up to 50 pounds.
  • Answer call lights in a timely manner.
  • Keep current on First Aide/CPR.
  • Displaying a “customer service” attitude toward patients, visitors, and other staff members at all times
  • Performs other duties as assigned by supervisor or any other management staff.

  • Current LVN or CNA License in the state of Texas
  • Minimum of one year experience as an LVN or CNA in a supervised setting
  • Current BLS CPR Card
  • Proof of negative TB or Chest X-Ray within the last six months
  • Immunization or Titer Record (MMR, TDap, and Varicella)
